— WHAT IS HAZOP —
A HAZOP (Hazard and Operability Study) is a structured, systematic examination of a process design to identify hazards and operability problems. Conducted by a multi-disciplinary team using guide words applied to process parameters, HAZOP is the most widely used Process Hazard Analysis (PHA) methodology for chemical and process industries worldwide — and is mandated by MSIHC Rules 1989 for Gujarat industries handling hazardous chemicals above threshold quantities.

Major Hazard Installations (MHI) handling Schedule 1 chemicals above threshold quantities must conduct HAZOP or equivalent PHA. DISH Gujarat inspects MHI compliance including current HAZOP documentation.
MoEFCC EC Compliance Requirement

Collection and review of P&IDs, PFDs, process descriptions, operating procedures, MSDSs, equipment datasheets, and MSIHC consent conditions. Node division based on process sections, operating phases (startup, normal, shutdown, emergency), and utility systems.
Formation of multi-disciplinary HAZOP team including process engineer, safety specialist, instrumentation engineer, operations representative, and maintenance lead. Prism's HAZOP facilitator leads the structured examination to ensure systematic coverage of all deviations.
Systematic examination of each process node using standard guide words (No, Less, More, Reverse, Other Than, As Well As, Part Of) applied to process parameters (flow, pressure, temperature, composition, level) to identify all plausible deviations from design intent.
For each identified deviation, the HAZOP team systematically identifies all plausible causes (equipment failure, human error, utility failure, external events) and consequences (fire, explosion, toxic release, environmental damage, business interruption). Risk ranking applied per client matrix.
Existing safeguards (IPLs — Independent Protection Layers) reviewed for adequacy against each identified scenario. Where risk exceeds acceptable level, actionable recommendations generated — design changes, additional instrumentation, procedural controls, or SIL-rated safety functions.
